If your chief complaint involves a skin condition, please also include a photo of the problem area of your skin. You will then receive an email back from me (Jamie Koonce) with all assessments for you to complete and send back via email. If you prefer to print all documents and mail them to me, please indicate this in your email and I will give you the address to mail it to.

- Comprehensive health assessment (49 page in-depth questionnaire).
- Nutrition Assessment (18 page in-depth analysis).
- 4 One-on-one phone coaching sessions with Jamie Koonce, MS, Dipl. OM.
- 8 Weekly coaching e-mails to keep you on track with recipes, health tips, and fitness exercises.
- Ordering of labwork when necessary (functional diagnostic tests usually between $49 - $175).
- Facial Diagnosis and Tongue Diagnosis (distance clients send photos).
- Interpretation of underlying imbalance(s) and recommendations based on all your health data.
- (Please note: Functional diagnostic tests are available for clients throughout the USA. You will either receive a mail-in home test kit or you will go to your nearest LabCorp facility for bloodwork. Test results are usually received within 24 hours.)
